Systems Point of Entry serves children with special health care needs and their families through referral and coordination of service. Family’s/children’s needs are identified and referrals are made to health and social service programs internal and external to OMCFH.

There are no family income limitations for receiving care coordination services from SPE. However, individual program guidelines apply when referrals are made to other programs.

The project also serves as a catalyst for systems change, as evidenced by the on-going partnership between the Social Security Administration/ Disability Determination Section and the OMCFH in facilitating SSI applications for disabled children. Some of those actively participating in this effort are SSA/DDS "Ticket to Work"; Department of Education with representatives from Head Start and Special Education; Division of Rehabilitation Services; West Virginia Advocates, etc.
