DOCUMENTATION
How many times in your professional career have you heard the phrase,
“Document, document, document!” or “If it’s not documented, then it didn’t happen.”
All services provided to our families must be documented. WV Birth to Three has
designed standard documentation forms that must be used when recording the
provision of a service. For example, your evaluation and assessment reports serve
as documentation of the results of the evaluation/assessment process. The
Individualized Family Service Plan (IFSP) is a legal document between WVBTT
a nd a family detailing the services that will be provided to them for their child.
Direct services are documented on intervention and service coordination activity
notes. Service coordinators use a variety of WVBTT standard forms to obtain
consent, release and gather information and to support the transition of children out
of WVBTT. Missed appointments and non-face to face activities are also recorded
so that there is clear documentation of the child and family’s time in WV BTT.
