The Individualized Family Service Plan (IFSP) represents a collaborative partnership
between the family and their early intervention team. This partnership begins with the
Family Assessment, incorporating the information gathered as part of the evaluation
and assessment process and ends with a decision as to how early intervention can
assist the family in enhancing their child’s development and participation in the
everyday activities and routines that are important to them.

The IFSP is a legal document describing the child’s eligibility status and the early
intervention service commitments for the eligible child and family. Identified service
needs, including the intensity and frequency of service, are based on assessment
information, current research, the family’s identified priorities, concerns and need for
support and information.
