Evaluation/Assessment
Reporting
Each child coming into the WVBTT System must have an evaluation/assessment
conducted to ensure they meet our program eligibility criteria.
WVBTT uses an Evaluation/Assessment Report Template for all disciplines: if you
are a nurse, you will use the Nursing Assessment; if you are a Nutritionist, you will
use the template designed for your profession. All templates have been designed by
your peers and address the necessary information needed to complete your report.

WVBTT has specific guidelines and criteria that must be met for eligibility. The 2.0
Definitions of Developmental Delay is also included in this section.
