This reporting system is provided for your convenience to report suspected instances of child abuse or neglect that do not require an emergency response. An emergency is a situation where a child appears to face an immediate risk of abuse or neglect that could result in death or serious harm. If you believe the child is in immediate risk of death or serious harm, please call 911. Then call Centralized Intake at 1-800-352-6513 instead of using this online referral form.

Please call Centralized Intake to report any child death or near-death situations that are from suspected abuse or neglect. To report suspicions of ADULT abuse or neglect, please call 1-800-352-6513, option 3.

Please answer all of the questions below to the best of your ability. A response in all of the boxes is required in order to submit the form. If you don't know the answer to a question, write "unknown" in the box. You may use the space to explain your lack of knowledge about that question.

If you believe that the child/children is/are in immediate danger, as defined by WV Code § 49-1-201, report immediately by phone to 1-800-352-6513, option 2.
