Medical Appointment Cards

WVCDHH has developed appointment cards for you to give your doctor's office to explain that you need an interpreter for your appointment. The first card explains your rights and tells the doctor's office how to get an interpreter. Give this to your doctor's office at your first or next appointment. If you go to the same office again, use the reminder card to remind them to get an interpreter for your next appointment. Before you give either card to your doctor's office, fill in the information on the front with y our name, the doctor's name, and your appointment date and time.

If you want paper copies of these cards, please contact WVCDHH. The links below will take you to cards that have been arranged so they will fit on regular paper so you can print them at home. You may also suggest that your doctor's office look on the website to get the information.
