PUBLIC HEALTH AWARDS
Public Health Awards Form
General Information
Our organization confers awards every year honoring selected
individuals for outstanding meritorious service in the field of Public
Health.
These awards may be granted to:
- Persons employed in health-related fields (Public Health Merit
Award),
- Organizations or groups concerned with Public Health
(Organization or Group Award), and
- Persons not related to the health profession (Citizens Award).
Listed below are the eligibility criteria
for award candidates. A separate application form should be completed
for each nomination.
Eligibility
- Two awards may be given each year for outstanding and
meritorious service in the field of Public Health, one of which
shall be given to a person employed in a health-related field and
one given to a person in a field not related to a health profession.
- The work of the individuals chosen shall have made an
outstanding contribution to the health of the population.
- Potential recipients should qualify in at least one of the
following categories:
a. A native West Virginian who has done outstanding work in the
state.
b. A native West Virginian who made their major contribution in
another state.
c. Anyone who has done outstanding work in the state, or mainly
in West Virginia.
- The honor shall not be restricted to long tenure, or advanced
age; but if given to a young individual, the merit of the
contribution must be such that it cannot be questioned.
- Recipients should be acceptable to the majority of their
co-workers in their respective field.
- Candidates for the honor shall be considered each year, and the
awards made only if justified.
SECONDARY CONSIDERATIONS:
- Regional or national recognition is desirable.
- Honor may well coincide with a landmark in the honoree's life.
- Groups may be considered as an option to an individual award.
For example: Lion's Club, 4-H Club,
etc.
Nominations
Nominations may be made by any individual
or organization in West Virginia by completing a
nomination form
and
submitting it by

WVPHA is interested in honoring individuals and groups that have
made public health contributions in West
Virginia. All information will be
kept strictly confidential.
