      Telephone Devices for the Deaf and Hard of Hearing and special attachments are supplied to eligible persons on extended loan. If this equipment is not being used in conjunction with the WV Commission for the Deaf and Hard-of-Hearing to assist the individual in need, it must be returned to the WV Commission for the Deaf and Hard-of-Hearing.

      If the individual moves out of the State of West Virginia the equipment MUST be returned. All equipment, if damaged or defective (not in working condition), should be returned to the Commission for repairs or replacement.  Click here for Certifying Authority Form.
