To apply for a Medicaid card, a pregnant woman must complete the WVKIDS-1 form (7/1/03). For convenience, these application forms have been distributed to primary care centers, rural health clinics, local health departments, physicians’ offices, and hospitals. The pregnant woman must complete the application and mail it or take it into her local Department of Health & Human Resources office with documentation of the following:

  1. Proof of West Virginia residency.

  2. Proof of citizenship.

  3. Written verification of pregnancy and estimated date of confinement (EDC).

  4. Verification of the pregnant woman’s family income.

  5. Social Security numbers for all family members.
