Eligibility for Infants

To be eligible for RFTS services, an infant must:

  1. Be a West Virginia resident and

  2. Have a valid DHHR Medicaid card Click here to apply online.

  3. Be less than one year of age

Medicaid eligible infants may be referred to the RFTS Program by a physician, nurse practitioner, nurse, social worker, other individual or parent/guardian because of medical, social and/or environmental factors.

All infants with a very high birth score who are Medicaid eligible are referred to RFTS for care coordination services.
