Online Users for Nurse Aide Verification
This notice describes the type of privacy when accessing the Online
Verification of Nurse Aides Web site.
Data Collection
The personal identifiable information (nurse aide’s name and the last four
numbers of the social security number, or the registration number)
provided by you will be used only in connection with verifying the status of
a nurse aide. The information provided by OHFLAC for verification is the
nurse aide’s name, registration number, date of registration, expiration
date, and basis of the registration.
OHFLAC uses session cookies when individuals access this site. We do
not use persistent cookies.
A cookie is a small piece of information that is sent to your browser when
you access a site. There are two kinds of cookies, session cookies and
persistent cookies. A session cookie is a line of text that is stored
temporarily in your computer’s memory. Because a session cookie is
never saved, it is erased from the memory of your computer as soon as
