Case Closure will adhere to the procedures set forth for the RFTS Newborn Hearing Screening Project. In all instances, the date, reason for closure and Designated Care Cordinators (DCC's) signature will be recorded on the Newborn Hearing Screening Project Follow-up form (HS001)

Note:The DCC must make three (3) unsuccessful attempts to establish contact/complete newborn hearing screening protocols before case closure. All attempts must be documented on the newborn hearing screening progress note (HS003).
