Title V Block Grant 2009 - The public is hereby notified that the Department of Health and Human Resources, Office of Maternal, Child and Family Health, is posting the Federal application to the Health Resources and Services Administration under the U.S. Department of Health and Human Services for Title V Funds. The FY 2009 application is available for review at the Office of Maternal, Child and Family Health, or online at www.wvdhhr.org/mcfh/blockgrant from October 1, 2008 through April 30, 2009. Persons wanting to submit written comments on the State's FY 2009 application in preparation for the FY 2009 application may do so by email to kathy.g.cummons@wv.gov or by mail to: Kathy Cummons, Director, Research, Evaluation and Planning, Office of Maternal, Child and Family Health, 350 Capitol Street, Room 427, Charleston, WV 25301, prior to June 1, 2009.
