• Proof of Identity:  WV driver’s license issued AFTER July 1, 2003.WV driver’s permit or photo ID card that is expired less than 6 months. An original, valid, current U.S. Military ID Card. An original, valid, current Certificate of U.S. Citizenship with photograph. An original, valid, current Certificate of Naturalization with photograph.  An original, valid, current U.S. passport with current photograph.

  • Are you a WV resident? In order to qualify for assistance, you must provide proof that you are a current resident of WV. Residential rental and/or lease agreement;  WV utility bills with service at a WV residence address;  WV property tax bill or receipt;  WV mortgage documents; WV homeowner’s insurance documents; WV W-2 Form less than 18 months old with applicant’s name and WV residence; WV weapons permit;WV Motor Vehicles registration card; WV Voter’s registration card; WV school enrollment IF applicant is under age 18 (must include applicant’s name and address); WV Homestead tax exemption

  • Proof of U.S. Citizenship:  You must provide proof of citizenship for all children who will be in child care. The most common proof of citizenship used is a certified, original Birth Certificate.

  • Are you working? If you are working, you will need to bring in pay stubs for the most recent month. Pay stubs  must reflect 1 full month of employment.  **If this is a new job, Choices will give you an Employment Verification form.

  •  Are you going to school? If you are attending school, we need your current schedule in the form of a tuition bill.

  •  Are you participating in WV Works? If so, bring a statement from your WV Works caseworker that states your WV Works check amount and your activity location & schedule.  

  •   Are you registered to receive child support? If you are registered to receive child support—even if you aren’t getting it at this time-- you MUST include verification. For more information on obtaining this verification, click here, or visit www.wvdhhr.org  If you have private pay arrangements for your child support, or special circumstances, contact your caseworker at Choices to see what you need to turn in.

  • Are you receiving any other kind of income? If you get SSI, military pay, or any other income, you will need to verify   this by turning in a current statement or check stub.
