Direct Deposit For Providers
Effective January 1, 2007, all child care family providers, centers, and facilities will be able to have their checks direct deposited. In order to participate in direct deposit, providers must:
ü Have a FACTS PLUS account (explained below)
ü Have a checking or savings account
ü Complete the direct deposit application; the form called the “FACTS Vendor Agreement.”
ü Attach a blank check to your completed direct deposit application and mark it “Void.”
ü Return your completed application with the attached blank, voided check attached, to:

ü Your application will be processed within approximately seven (7) business days. Your first direct deposit should be received within 60 days after your application is processed.

*FACTS PLUS: Once you are set up with direct deposit, you will no longer receive a pay stub with an itemized statement. You can view your payment information online at the FACTS PLUS website.

PLEASE NOTE: Your direct deposit paperwork MUST be sent to the address above. If you send the paperwork to Choices, we will return it to you, thus causing your application process to be delayed.
