We are located to the right of the lobby. If no one is at the counter, please ring the bell, and someone will be with you shortly.
What You Will Need
- Identification is Required!
We accept any one of the following forms of Identification:- State Issued Driver's license (can be expired)
- United States, or Foreign passports
- Military ID
- Photo-ID cards from a government sponsored program
- Major Credit Card
- Motor Vehicle Registration card
- State Social Services program card
- A recent utility bill in the name of the person who is requesting the certificate.
- The Proper Fee
Our fee is $12 per certificate. We accept cash, checks, and money orders, but we do not accept credit or debit cards, but there is an ATM machine in the lobby, next to our office. - The Proper Information
For Birth certificates, that means the person's- Name as it appears on the birth certificate.
- Date of Birth
- Mother's full Maiden Name and the Father's full name
- The city and county of birth
- Name as it appears on the death certificate.
- Date of Death
- City and county of death
- The Full Maiden Name of the Bride
- The Full Name of the Groom
- The City and County of marriage
- the County from which the marriage licenses was obtained, if different
- Name as it appears on the birth resulting in stillbirth certificate.
- Date of Delivery
- Mother's full Maiden Name and the Father's full name
- The city and county of delivery
You may either fill out an application are found here, or wait until you arrive at our office to complete an application.
Please Note:
If the Person who will be picking up a birth certificate is not entitled to the certificate, it may be possible for that person to obtain the certificate if a permission slip is completed and faxed into our office.
A permission slip must be filled out by one of the parents or by person who is named on the birth certificate. A legible copy of a valid state driver's license must accompany the permission slip. The permission slip must also be signed, dated, and name the person who is picking up the certificate.
