Correction to a Death Certificate for a person who has been dead less than
one year
The funeral home who is handling the case should fax a letter to Vital Registration requesting a correction. The request must be specific, dated, and signed by the funeral director (or authorized staff) with the name printed or typed below the signature.Note: Correcting the marital status requires submitting an affidavit to correct death certificate, signed and notarized, by mail to Vital Registration's Registration Unit.

Correction to a Death Certificates for people who have been dead one year or longer
Either the informant (the person listed on the death certificate as the one who supplied to the funeral home the names, address and other information), or the funeral home that is listed on the death certificate may submit an affidavit to correct death certificate.
