Title	Advocating for Chronic Disease Management and Prevention 2011

Resource Description:	The Division of Health Promotion and Chronic Disease presents its strategic to plan to address chronic disease in a more comprehensive approach. This report presents data of the current state of chronic disease in West Virginia as well as socioeconomic and behavioral risk factors that contribute to the development of chronic conditions. The integrated approach addresses the fact that many individuals in our state suffer from more than one chronic condition and have than one risk factor for these diseases.
