I. PURPOSE:

This section provides guidance to hospital personnel who are responsible for completing the NICU referral form. This process will provide information to the infant’s Right From The Start Care Designated Care Coordinator in order to develop a comprehensive family service plan for the infant’s first year.

II. CRITERIA FOR REFERRAL:

Only those infants who are eligible for WV Medicaid or SSI will be referred to The Right From the Start Project for care coordination services. Each of the following hospitals (Women and Children’s, Ruby Children’s, and Cabell Huntington) will refer to the Birth Score Office all Medicaid/SSI eligible NICU/Special Care infants who meet one or more of the following criteria:

1. High Birth Score

2. NICU/hospital length of stay > 10 days.
