Completing the New Teaming Authorization Process

WV Birth to Three will be implementing a new process for authorizing teaming services, effective October 1, 2008. This flyer announces a series of conference calls for the field on the “Completing the New Teaming Authorization Process”. The series will be facilitated by WV Birth to Three State staff and provide information about these new requirements. Time will be allowed at the end of the call for questions and answers related to the form and process.

Through participation on the call individuals should increase their knowledge of:

WV Birth to Three will be issuing three (3) contact hours for this call. To receive the contact hours, participants must call in 10 minutes prior to the start of the call for roll call. Roll call will also be taken at the end of the call to verify participation. Participants who call in after roll call has been taken, or exit the call early, will not receive contact hours.

Participants will need to download the following materials in preparation for the call: 1) Practitioner Confirmation form – revised 6-01-08, 2) Teaming Authorization form – 10-01-08. These forms will be posted in the Standard Documentation/Forms link on the WV Birth to Three website, prior to the first call.
