FINANCIAL ELIGIBILITY CRITERIA FOR THE MR/DD WAIVER PROGRAM

CRITERIA FOR FINANCIAL ELIGIBILITY

INCOME
-
The individual's monthly income may
not
exceed 300% of the current maximum Supplemental Security
Income (SSI) monthly payment. The maximum SSI monthly
payment may be found by contacting the local county DHHR
office or local Social Security Administration office.

The parent's income is
not
considered for determining financial eligibility.

An individual
does not
have to be SSI eligible to become eligible for
the MR/DD Waiver Program.

ASSETS
-
An individual's assets may
not
exceed $2,000 in order to be Medicaid eligible for
MR/DD Waiver services (
excluding residence and furnishings
).

The parent's assets are
not
considered for determining financial eligibility.

APPLICATION
-
The application for financial eligibility is
completed at the county DHHR office by the applicant,
a family member, or legal representative.
-
It is the responsibility of the Service Coordinator
to ensure the application for financial eligibility
is completed at the county DHHR office.

Whoever completes the application process at
the county DHHR office
must specify
that this is
an application for the MR/DD Waiver Program
.

An Economic Service Worker at the local/county DHHR
office will complete the application for financial
eligibility on an ES-2 form. It is the responsibility of
the county DHHR office to make financial eligibility
determinations.

Once an individual has been approved for MR/DD Waiver
services by the State MR/DD Waiver office, it is the
responsibility of the
Service Coordinator
to notify the county DHHR Economic Service
Worker to
change the code
on the Medical Card to "MR/DD Waiver."

All eligible MR/DD Waiver participants, who are not
otherwise eligible, for a Medical Card will receive one.

RE-ESTABLISHING FINANCIAL ELIGIBILITY

MR/DD Waiver participants who are not
otherwise eligible for a Social Security
Income (SSI) will have to have financial
eligibility re-established every six
mionths. This is done at the local/county
DHHR office where the person resides. It
is the responsibility of the Service
Coordinator to ensure this is done
on an annual basis.

MR/DD Waiver participants who eligible
for Social Security Income (SSI) without
being a participant of the MR/DD Waiver
Program must have financial eligibility
re-established annually by the Social
Security Administration.
