Applicants
will be considered for admission if:
The applicant is a resident of West Virginia
The applicant's condition requires care/treatment
above the level of room and board, and below the level of care provided by a Skilled
(Medicare) Facility; primary problems are medical and/or nursing in nature; an approved
PAS-2000 is available; care needs can be met through facility programs or through outside
resources; and use of mechanical or chemical restraint is NOT
required.
The applicant is willing to sign the Admission
Contract and has capacity to make medical care decisions; OR the
applicant's legal representative will sign on behalf of the equipment. |

Application
Procedure:
Under Federal OBRA regulations, all applicants
for admission to any nursing facility accepting Federal funds must undergo Pre-Admission
Screening. Any hospital, most physicians, and any DHHR office, can provide the
PAS-2000 form
required.
