What should I do if I notice problems in a home/facility?

Where you would report concern depends on the nature of your concern and the type of placement setting. Some general guidelines follow:

• abuse or neglect of a resident - report to Adult Protective Services by calling the local Department of Health and Human Resources or the statewide Hotline at 1-800-352-6513.


• poor care, unclean facility, unsafe facility, etc - report to the licensing/certifying agency






Nursing Homes - OHFLAC, Long-term care Ombudsman




Personal Care Homes - OHFLAC, Long-term care Ombudsman




Residential Board and Care Homes - OHFLAC, Long-term care Ombudsman




Adult Family Care Homes - Local Department of Health and Human Resources




Emergency Shelter Care Homes - Local Department of Health and Human Resources







• violent crime - (rape, aggravated assault, assault & battery, etc - local law enforcement

Whenever in doubt, however, you may call the local Department of Health and Human Resources for assistance.
