Who is Eligible?
Adult Family Care Homes
The Adult Family Care Program is available to adults who are no longer able to remain in their own home and require an alternate living arrangement due to physical, mental, or emotional limitations. This program is primarily designed for:
- adults who have no relatives or families
- adults whose families are unable to adequately provide for them or unable to accept this responsibility
- adults who need a protective gap between independent living in the community and a more restrictive facility setting such as personal care home or nursing home
- adults who are unable to care for themselves in their own home but are ambulatory and able to perform personal care tasks and home management tasks with some supervision and assistance
Eligibility for placement in this type of setting is not limited by the type and amount of income the adult receives. Rather, Adult Family Care is open to anyone who is in need of this type of service and meets the qualifications listed below. All placements in Adult Family Care Homes, however, must be made or approved by the Department of Health and Human Resources. While eligibility for placement in Adult Family Care Home is not effected by the adult's income, payment by DHHR for placement in Adult Family Care is affected by the amount of income received by the adult and the level of assets they have available. In order to be eligible to receive Adult Family Care services, the individual must meet at least one (1) of the following criteria:
- he/she must be age sixty-five (65) or older
- he/she must be at least eighteen (18) years of age and have an established disability or a disability may be established by a thorough evaluation and documentation of the person's condition by a licensed physician and a determination by the social worker that this medical evaluation does indicate the need for supervised care
- he/she be at least eighteen (18) years of age is currently receiving Adult Protective Service or APS Preventive Services from the Department of Health and Human Resources.
In the case of eligibility based on an active APS or APS Preventive Services case, Adult Family Care must be needed to eliminate the abuse, neglect or exploitation that was verified during the APS investigation. In addition, the adult must be ambulatory, continent, non-combative, and free of communicable disease.
Emergency Shelter Care Homes
Emergency Shelter Care is available to adults who are no longer able to remain in their own home and require placement on a short-term, emergency basis while alternate arrangement for their care and safety are being made. Eligibility for placement in this type of setting is not limited by the type and amount of client income. All placements in Adult Emergency Shelter Care Homes, however, must be made by the Department of Health and Human Resources. In order to be eligible to receive Emergency Shelter Care services, the adult must meet at least one (1) of the following criteria:
- he/she must be age sixty-five (65) or older;
- he/she must be at least eighteen (18) years of age and have an established disability or a disability may be established by a thorough evaluation and documentation of the person's condition by a licensed physician and a determination by the social worker that this medical evaluation does indicate the need for supervised care
- he/she be at least eighteen (18) years of age is currently receiving Adult Protective Service or APS Preventive Services from the Department of Health and Human Resources.
In the case of eligibility based on an active APS or APS Preventive Services case, Adult Emergency Shelter Care must be needed to eliminate the abuse, neglect or exploitation that was verified during the APS investigation. In addition, the adult must be ambulatory, continent, non-combative, not a dander to themselves or others, and free of communicable disease.
Residential Board and Care Homes
Eligibility for placement in a Residential Board and Care Home is determined by the level of care required by the adult being placed and the facility's ability to meet those needs. In situations where placement is being arranged by the Department of Health and Human Resources, the provider may be eligible to receive a supplemental payment from the Department for the adult's care. In order for the Department to consider a supplemental payment, the adult must meet at least one of the following criteria:
- he/she must be age sixty-five (65) or older;
- he/she must be at least eighteen (18) years of age and have an established disability or a disability may be established by a thorough evaluation and documentation of the person's condition by a licensed physician and a determination by the social worker that this medical evaluation does indicate the need for supervised care
- he/she be at least eighteen (18) years of age is currently receiving Adult Protective Service or APS Preventive Services from the Department of Health and Human Resources.

Eligibility for placement in a Personal Care Home is determined by the level of care required by the adult being placed and the facility's ability to meet those needs.
