This is an online survey to support accurate reporting of nursing home outbreaks. This survey will collect data for nursing homes with active COVID-19 outbreaks. The data reported will be used to populate the data seen on the online dashboard for nursing home outbreaks at www.coronavirus.wv.gov.
Data should be reported weekly every Thursday by close of buisness until the outbreak has been closed by public health. The nursing home dashboard data will be posted weekly on Fridays.
